Do All NOD, LIS PENDS, Etc Get Sent / Filed
at the same time, or rather at the same point of "distress" in the forclosure process? Or does one lender wait until the borrower is 4 payments behind, another 3, a third 5 payments etc.

Lender specific. Some are very patient, others file as soon as they are legally able.
The times when the lendor can file will be state dependent.
