There is nothing magical about a boilerplate form LOA (letter of authorization). A handwritten letter on looseleaf paper will work just fine.

The LOA is from the mortgagor (Borrower) to the mortgagee (Lender). Make sure all names and addresses, and mortgage loan account numbers are accurate and easy to read.

Dear Lender: Please accept this as your written permission to discuss my mortgage loan account # _____________(and possible non foreclosure alternatives) with ________________.

    Agreed. I've used 5 different versions of authorization forms, from overly detailed sheets to a scawled on napkin. The banks don't care - all they need is 2 sentences.
