Cheapest and fastest way usually is for the LL to join the local Rental Association and get credit reports throught them. They're already set up with some credit firm so you don't have to reinvent the wheel.

Thanks for the responses. I ended using a standard form from Staples, getting it filled out and signed and then sending it to equifax.
