You can make one yourself. It has to specify that the person whose name is on the loan is allowing you permision to seek info OR for the bank to release/discuss loan related info with YOU on their behalf. It also has to be signed by the mortgagee, include the SSN, Loan #, current mortgagee and bank addresses and obviously your name. Just create a form in WORD.
