"Service of Notice" form?

Anyone have a "Service of Notice" form? Or an example of one. I have to have the Sheriff serve this form to all parties that have interest on a property that I hold the Tax Deed to. And I don't know where to get this form. Any help would be appreciated. :-S

    Try Your local County Courthouse and ask for the list of Free forms offer.

    See if the Notice of Service form is listed there!
