    What exactly do you mean by seller's authorization form? Do you mean a "disclosure" authorization that outlines the terms of the short sale attempt between you and the homeowner?

    Don't get hung up on boilerplate forms...

    A simple authorization from the mortgagor to the mortgagee (allowing them to speak with you about the loan, and possible non foreclosure alternatives) can be written in crayon on a napkin.

    I use the following verbiage:

    Authorization to Release Information
    Dated: _________________________
    TO: __________________________________________

    Borrowers: __________________________________________

    Property: __________________________________________Loan Number: _______________________

    We, the undersigned, hereby authorize you to release information regarding the above-referenced
    loan to __________________________________________

    This form may be duplicated in blank and/or sent via facsimile transmission. This authorization is a
    continuing authorization for said parties to receive information about my loan, including
    duplicates of any notices sent to me regarding my loan.

    SSpro is right , Keep it simple, All you need to say is . I ------ give my permission for " Bank name" to release any any all info. on " prop. addr." to " Your name.
