Help With The Sales Contract

Im working on my first SS deal and I needed a little help. Being new to this and never having purchased a property before, I am having trouble filling out the sales contract to be submitted to the bank for short sale consideration. I have got several various forms of blank purchase contracts but I do not know what exactly needs to be completed. Do I just fill in the total purchase price and have the owner sign it and deal with all the particulars later? Will this be enough? Do I need some verbage making the contract conditional on SS approval? Does anyone have a contract form that they use for Short Sales that they could email me?
