I have used AIA contracts on many jobs. They are straight forward and balanced between contractor and owner. They have contracts for small - med jobs, change orders, release of leins etc.
They are available in software format or paper format. For less than $80 you can obtain copies of the 3 or 4 documents you need.
the key is to be real specific about what gets done, how, where and by when. I really would suggest you get an attorney for this one to review everything. Where I live, many contractors come across as great people until you find out about their drug / gambling / booze problems. No kidding, there are a lot of lowlifes out there who will try and take advantage of you.

Lack of a clear and concise exit strategy, rehab plan and timetable will cost you money. It will cost everyone money....
I think that going into a rehab project without clear concise goals is like playing russian roulette, you may pull the trigger a few times without consequence but sooner or later you will get creamed.
Make no mistake, it is about the money.
It may not be about "how much" beyond your monthly needs, but it is about the money.
All you have to do is ask yourself "would I be doing this if I made zero money or lost money on every deal?"
I myself resist structure and have had to teach myself to embrace goal setting, planning, and timetables. Without these, real estate would just be a haphazard hobby like my waterskiing (which costs me money)...

Currently with 4-5 rehabs going at a time I have to have a clear plan
