Sandwich Lease Option Forms?

Hi, Can anyone tell me where I can find these agreement forms? It must be subject to me finding a tenant-buyer to occupy the property. Also, the form must give me space to put in a clause that the tenant-buyer is responsible for repairs & maintenance under $200...and the seller for r & m over $200. Please let me know where I can find these forms for FREE...or purchase them online. Thanks.
