Setting Up An LLC--Questions

I working on gathering information and forms I need to set up my LLC. I'm in PA (Bucks Co).

I just got the "Certificate of Organization Domestic Limited Liability Corporation" form and the "Docking Statement" of the Sec of State's web page.
On the Docking Statement:

  • What do "Domestication", "Division" & "Consolidation" mean?
  • Should I leave "Specified Effective Date" blank or fill it in? What do each mean? if I leave it blank, does that make it effective immediately?
  • Where it says "Description of Activities" is "Real Estate Investments" sufficient? Or do I need to be more descriptive?
  • To simply things, I'm planning to make the fiscal year the same as the calendar year. Are there any reasons to do otherwise?


On the "Certificate" form:
  • What does "Name of Commercial Registered Office Provider" mean?
  • What does "A member’s interest in the company is to be evidenced by a certificate of membership interest" mean?


Also besides just the forms, how do I find out what kind of fees (both one time and recurring) will be involved with having this LLC here in PA?[*]What does "Management of the company is vested in a manager or managers" mean?
