Question about Auth. to Release Info form?

I have a critical question about the 'Authroization to Release Information' form I have before going on my first appointment today. I have posted my version of the form below. My question is, where the form says "To:" do I put my name and address there, or do I put the loan companies name and address there? A quick reply is appreciated. Thanks!

    The TO line is usually where you would put the lender name.

    Some investors dont use a TO line on their ATR form. You dont really need it because you will be sending a cover letter to the L/M rep anyway.

    If there is more than one loan on the property, I would reccommend that you use two seperate forms, instead of putting both loan numbers on the same sheet. If you are trying to short sale both the first and the 2nd, the banks may ask you less questions about the other lien.
