    A Letter of Authorization is written notification from the mortgagor to the mortgagee, giving permission to discuss with a third party (you) the mortgage loan account.

    You need to name the parties, identify the loan #, and insert the date.

    You don't need a form. All you need is a pen, paper, envelop and a stamp.
