Bird Dog Lead Sheet

Does anyone have a lead sheet that they give their Bird Dogs that they could post or email? I am thinking about working on lining up a few Bird Dogs and would like to supply them with a sheet to gather info so that I can keep it standardized. Thanks! Rob
